This form must be signed and returned to the SEMILLA CASAS Program Director before the participant is approved to participate in a SEMILLA CASAS Program. Health Screening Form, Part 2
All participants who plan to participate in a SEMILLA CASAS program for more than 30 days must be medically cleared to participate in SEMILLA’S CASAS Programs for an extended period of time. Copies of this form are to be retained by both the healthcare professional and the participant as a confidential medical record. Download the form located at the top of this page and fill it out.
